d40df57546 Replaced GNU make-specific stuff by the general make syntax to make 
the file more portable.

e28944fe2e - Removed all use of html.sty. 
- Removed all references to the CGI manager.

 - Removed autoconf support.

 - Removed the index.

 - Reformatted the document to look good and to compile without
   warnings.
